Basics - The essentials

This level can best be described as lifestyle elements comprising of:
1. Exercise:
Studies show that exercise does not have to be extremely vigorous or intense. Just 3 or 4 decent workouts each week- i.e., brisk walks, cycling, swimming or gym attendance for 40 minutes at a time, (and rotated so that you stimulate different muscle groups) can be very beneficial for physical condition, heart and mental outlook etc. Additional efforts, like climbing stairs and parking away from the shops can all make a difference. 

2. Eat and cook sensibly:
No single diet works for everyone, watch your calorie intake and understand carbohydrates, (i.e., simple and complex) and how they change when food is cooked differently, (e.g. boiled or grilled etc). Always choose organic and avoid all processed foods, avoid saturated and hydrogenated fats (i.e. animal and margarine) - instead choose mono unsaturated fats, the good oils include olive, canola, sesame, sunflower and walnut. If you really can’t do without sweetness then choose fructose or Xylitol over sucrose. Avoid foodstuffs that trigger excitotoxins, particularly aspartame and monosodium glutamate. Avoid eating the same things all the time and discover how to balance your body’s Ph through diet. Lower your estrogenic and toxin levels by not cooking anything in plastics and saran wraps, (also avoid freezing in the same). Some also believe that food cooked in microwaves should be avoided, but certainly all aluminum as well as non-stick pots, pans and cutlery should be eliminated from your kitchen. 

3. Drink water regularly:
Consume 2 to 3 liters of pure water per day, (spring water in glass bottles). Do not substitute water for tea, coffee or beer etc., these drinks are diuretics, i.e. you will lose more water than you put in- especially true of coffee. Excess alcohol will inhibit your liver/ kidney function and lead to an accumulation of estrogens, (hence weight gain) but red wine (due to the presence of resveratrol and antioxidants) has been shown to be beneficial to blood/ heart health in small doses. As we dehydrate with age, this problem can be overcome by drinking water before you feel thirsty. 

4. Be positive:
Maintain a loving, social and active persona to have a positive outlook. 

5. Relax:
Chill out! Whether it’s meditation or by other means- do what’s best for you to regularly reduce your stress levels. 

6. Get some sunshine:
Around 30-minutes of sunshine a day can dramatically improve vitamin D levels. This has many benefits, e.g., a recent study highlighted that men with higher vitamin D levels had half the incidence of prostate cancer. But remember that over doing sun/ UV exposure can lead to skin cancers and accelerate the appearance of aged skin/ wrinkles through “photoaging.” A careful balance needs to be maintained. 7. Maintain a healthy sleep pattern:

    Be aware of what your allergic reactions. Everyone has some allergies to varying degrees, the most common are those associated with pollens, dairy produce and other foodstuffs such as wheat, coffee, food colorants etc. The “hidden” allergies are those linked with pesticides, plastics and metals, (particularly mercury and nickel). Sunscreen

Sunscreen is usually the last thing you apply as part of your skincare routine (or at least it should be!). We’ve chosen to talk about it first as we feel it is the most important product you can use to prevent aging (other than Botulinum toxins- which you should compliment by using sunscreen anyway). 

Over 90% of sun damage is caused by incidental exposure such as the commute to work, short walks etc! UV radiation can lead to sunburn, accelerate skin ageing and stimulate skin discolouration (freckles and dark patches on the skin or age spots), so it is important to wear sunscreen everyday!
 
The most important thing to know about sunscreen is to only purchase a sunscreen which clearly states on the label that it is Broad Spectrum or has Multi Wave protection. If the sunscreen is cheap and does not say either of these on the labels, there is a good chance that it will only block out UVB rays (which will not protect you from skin cancer which is caused by UVA and UVB rays!)

Sunscreens come in 3 different forms:
-  Chemical- Neutralise the UV rays within the skin
-  Physical- form a physical barrier between your skin and UV rays (quite thick)
-  Hybrid- combine physical and chemical sunscreen (thinner in consistency to physical barriers)

Whilst a sunscreen and moisturiser combination is better than nothing, the moisturiser may dilute the sunscreen content and not provide you with the SPF protection stated on the packaging. So wherever possible use a moisturiser and separate sunscreen of at least SPF15. A broad spectrum SPF 15 is often lighter on the skin than SPF 30 and provides 95% protection from UV rays compared to 97% for SPF30. Remember to apply your sunscreen 30 minutes before leaving the house!



Tyrosinase Inhibitors

Tyrosinase Inhibitors are used to treat all forms of pigmentation of the skin, other than those caused by genetics.  They can act on other internal sources, such as Hormones and medications (such as the contraceptive pill) and external sources such as UV exposure and Post Inflammatory Hyperpigmentation. Tyrosinase Inhibitors work by blocking or sedating the production of melanin which causes your skin to have dark spots.

The most common over the counter Tyrosinase inhibitors are from plants (such as ascorbic acid-Vitamin C, Mulberry extract, Kojic acid etc). If over the counter medication is not doing enough to reduce your spots, doctor’s can recommend and prescribe stronger creams such as Hydroquinone (banned in Australia) or Azelaic acid.

It is advised that all skin undergoing resurfacing or rejuvenating (eg./Chemical peels, IPL, Laser Photo Dynamic Therapy) be prepped with Tyrosinase inhibitors to reduce the risk of post-inflammatory hyperpigmentation.

Method of use:
Tyrosinase inhibitors should be applied twice daily for a minimum of  2 months. It is imperative to wear a broad spectrum sunscreen whilst using Tyrosinase inhibitors.


Alpha Hydroxy Acids (AHA)

Alpha Hydroxy Acids (AHA) are a group of water soluble naturally occurring acids found in nature and the human body.

Primary benefits:
  • exfoliation (unlike most scrubs the AHA’s penetrate the skin to target dead cells and level off the peaks and troughs of the skin from within the skin. Scrubs simply rub off the top of the peaks without altering the skins surface)
  • hydration (AHA’s stimulate Hyaluronic acid within the skin which provides moisture and resilience to the skin
An added benefit of AHAs is that they only target dead cells.

Best to use:
  • lactic acid- (less aggressive and more hydrating than glycolic acid) and
  • glycolic acid.
Beware using AHAs other than lactic and glycolic. You may find you are spending money for nothing as any other AHA molecule is often too large to penetrate into the skin so will provide little to no benefit.
It is also important to consider the following when choosing an AHA:
  • Concentration: 7-14% is best
  • pH: pH3-pH4.5 any higher will provide no active benefit
  • Formulation: AHAs should not be contained in an oil based serum as they are a water soluble product. Should never be combined with retinoids (vitamin A derivatives) as the combination will de-activate the activity and potency of the retinoid.
Instructions for use:
You may feel a tingling sensation after application of an AHA. This is a good thing if the skin is tingling but staying the same colour. If your skin goes red after application, seek assistance from your skin specialist. Use for 6 months then just once a week / fortnight for maximum benefit.

 

Retinoids

Retinoids refer to any chemical compound related to Vitamin A (often known as the anti-aging vitamin). The main function of Retinoids is
  • Resurfacing (Retonoic acid has the ability to affect cell formation hence it’s reputation to reverse skin damage)
  • Exfoliation
 Retinoids act in a number of ways:
  • Promotes collagen production and elastin
  • Regulates cellular proliferation and differentiation
  • Increase epidermal thickness
Like with AHAs the concentration of retinoids has a great effect on the effectiveness of the treatment. Retinoic acid is available only with a Doctor’s prescription and is the strongest Retinoid. Retinol and Retinaldehyde (Retinal) are both available in direct to the public skincare at effective concentrations, and are considered the best to use for the average individual. Many skincare ranges use Retinyl Palmitate which is a much ‘weaker’ form of retinoid, so in order to gain maximum benefit, avoid these products in favour of those which use Retinal or Retinol.

It is also important to use a Broad Spectrum Sunscreen to protect your skin which can will become more photosensitive when using retinoids.

Method of use:
Each individual will have a different reaction to Retinoids. It may be necessary to build up tolerance to the retinoid over one to two months in order to avoid a reaction (rednesss etc). Use twice a week, building up to every alternate evening and eventually to daily use for 6-12 months. After this time you should either give your skin a 6 month break, or continue to use bi-weekly.

It is always advised to seek the advice of your doctor if you become pregnant and would like to continue using Retinoids. 

Vitamin C

Vitamin C is a versatile skin care ingredient as it promotes collagen synthesis, acts as an antioxidant and can be photo-protective. It can also act as a Tyrosinase inhibitor and lighten dark spots.

Beware… if your Vitamin C product has turned orange or brown, it may have oxidised and could actually do free radical damage! Many people find it easier to purchase a Vitamin C powder and mix it with a water based product to ensure it will work effectively.

All clients seeking treatment for pigmentation, telangectasia  (dilated blood vessels near the surface of the skin or mucous membranes) or aged skin should incorporate Vitamin C into their skincare regime. Smoking depletes the body of Vitamin C which is one of the reasons many smokers show accelerated signs of aging around the mouth and eyes, so it can be a very useful treatment these conditions also.

Prevention

If you don't wear a sunscreen every day, today is the day to start. Daily application of sunscreen with an SPF of 15 or higher- whether on its own, or in a moisturizer - will prevent additional damage to your skin. Also, studies have shown that daily use of sunscreen can actually reduce the existing number of actinic keratoses - abnormal skin eruptions, or precancers, that can lead to skin cancer.

Retinoids

Retinoids have been a breakthrough discovery for the treatment of photoaged skin. Synthetic derivatives of Vitamin A, they can improve discoloration of the skin, degeneration of elastic tissue, and fine wrinkling by enhancing naturally occurring production of collagen and elastic fibers. In addition to giving skin back its youthful appearance, retinoids can inhibit tumor growth, decrease inflammation, and enhance the immune system. Continuing use of retinoids can reduce the number and size of actinic keratoses. Currently, two retinoids are available in the US for treatment of photoaging: tretinoin (2) and tazarotene. Note that retinoids can irritate the skin and cause dryness and photosensitivity - an extreme sensitivity to the sun. They are available by prescription only.

Lasers

Lasers are used to vaporize the skin's sun-damaged top layer, leaving a softer, smoother skin surface with less pronounced wrinkles. With their pinpoint accuracy, lasers are ideal for sensitive skin areas such as around the eyes. The treatment can result in tenderness, scarring, redness and swelling that can last for several weeks while the new skin grows. Darker-skinned individuals can experience permanent loss of pigmentation.
 
Lasers can be used for treatment of actinic keratoses, as well. A new technology called IPL (intense pulsed light) is a non-skin-removing technique ("non-ablative") that offers patients a softening of lines and shallow wrinkles, fading of some scars, removal of broken capillaries, and stimulation of new collagen growth quickly. After-effects can include swelling, redness, blotchiness, and darkening of pigmented areas. Usually several sessions are needed for optimal results, and the full effects of the treatments may not be visible for up to six months after the final treatment.


Chemical peels

Chemical peels are often used to rejuvenate the skin, reduce wrinkles, and treat actinic keratoses. A doctor removes layers of skin with a mild acid solution. Depending on the damage, a light, medium, or deep peel is used. After the skin heals, a smoother, younger-looking outer layer forms. Light peels usually result in only a slight redness and photosensitivity, but deeper peels can result in significant swelling, redness, and photosensitivity that lasts for several weeks.

Dermabrasion

Dermabrasion uses a small high-speed, rotating metal brush or file to abrade the upper layers of the skin and smooth out surface irregularities. It is used, often with long-lasting results, for treatment of substantial wrinkling and leathering from the sun, pigmentation problems, and acne scars. Either local or general anesthetic is used during the procedure. Redness, swelling and pain are typical after treatment. Healing can take from 7 to 14 days.

For mild to moderate photoaging, a variation of the technique known as microdermabrasion uses tiny particles that pass through a vacuum tube to gently scrape away aging skin and stimulate new cell growth. Mild redness is typically the only side effect.

Botox

Almost 20 years have passed since the first injections of botulinum toxin A were done for frown lines between the eyebrows. Since then, it has become one of the most popular cosmetic procedures in the US. When skin loses its elasticity, facial wrinkles develop and form lines - like the pleats of an accordion. Botox works by weakening or temporarily paralyzing the muscles and smoothing the wrinkle away.

Botox is currently approved by the FDA only for the frown lines between the eyebrows. However, it is used "off-label" (without FDA approval) for horizontal forehead lines, crow's feet around the eyes, vertical lip lines and neck banding. Side effects are usually mild to moderate, and can include bruising around the injection area, headache and muscle weakness. Botox does not reduce the incidence of skin cancer. It is a cosmetic procedure only.


Fillers

Filling substances are injected under facial skin to round out contours and correct wrinkles, furrows and hollows. A number of substances are currently used in the US as fillers, including bovine collagen, polymer implants, and the patient's own (autologous) fat. To maintain the effects of bovine collagen and autologous fat, the procedure needs to be repeated periodically. Some patients are allergic to fillers, and a dermatologist should perform an allergy test before treatment. Side effects can include redness, swelling, bruising, and moderate pain, depending on the type of filler used.

Many of these treatments can be used in conjunction with one another to produce younger-looking skin and reduce the risk of skin cancer. It is important to consult with a licensed dermatologist to discuss treatment options, as many of these treatments can cause damage in inexperienced hands.

Breakthrough Research: Fish Oil Supplements Have Anti-aging Effects

 (NaturalNews) Despite downright silly and inaccurate headlines just a few weeks ago proclaiming that fish oil supplements rich in omega-3 fatty acids are a waste of money, now comes research that reveals the opposite. Taking enough of these supplements appears to have enormous health benefits -- including dramatic anti-aging effects.

Last month, Natural News told readers the truth about a much ballyhooed previous study which, according to the inaccurate reporting by mainstream media, meant taking omega-3 fatty acid supplements (almost always in the form of fish oil) was a waste of money and did nothing to protect health (http://www.naturalnews.com). In fact, the study was not the last word about fish oil supplements at all and even the researchers who conducted it didn't make that claim.

It was a meta-analysis of studies involving close to 70,000 people all lumped together and included research subjects who had only taken omega-3 supplements for a short while. Some took very little. Even the authors of the study noted that "...an individual patient data meta-analysis would be more appropriate to refine possible associations related to, among others, dose, adherence, baseline intake, and cardiovascular disease risk group." But that didn't stop the media from ignoring that important point and proclaiming, as ABC News did, that omega-3s are "not a lifesaver" and do nothing for health in general.

Now, a new study by Ohio State University researchers (just published online in the journal Brain, Behavior, and Immunity) found that overweight but healthy middle-aged and older adults who took a substantial amount (either 2.5 grams or 1.25 grams) of omega-3 supplements regularly for four months altered a ratio of their fatty acid consumption in a way that boosted preservation of tiny DNA segments in their white blood cells.

What does this mean specifically? These miniscule DNA bits are known as telomeres. They shorten over time in many types of cells as a consequence of aging. Stopping this shortening of telomeres has long been thought to be a key to halting the aging process or at least slowing it down.

The Ohio State scientists found that lengthening of telomeres in immune system cells was more prevalent in people who substantially improved the ratio of omega-3s to other fatty acids in their diet. What's more, the substantial and regular supplementation also reduced oxidative stress, known to be caused by excessive free radicals in the blood, by about 15 percent compared to the oxidative stress measured in a control group of research subjects who received placebos instead of real supplements.

"The telomere finding is provocative in that it suggests the possibility that a nutritional supplement might actually make a difference in aging," said Jan Kiecolt-Glaser, professor of psychiatry and psychology at Ohio State University and lead author of the study, in a media statement.

In yet another finding from this study, Kiecolt-Glaser's research team reported that omega-3 fatty acid supplements lowered inflammation in this same group of adults. This blows the assertion that fish oil has no heart benefits out of the water.

"Inflammation in particular is at the heart of so many health problems. Anything that reduces inflammation has a lot of potentially good spinoffs among older adults," Kiecolt-Glaser said. She added that people who are less healthy -- especially those who are under chronic stress -- may gain even more benefits from omega-3 supplementation.

The scientists also found that decreases in an inflammatory marker in the blood called interleukin-6 (IL-6) in the people taking omega-3 supplements were associated with telomere lengthening. In their earlier paper on omega-3s and inflammation, the same researchers had already reported that omega-3 supplements lowered IL-6 by 10 to 12 percent, depending on the dose. By comparison, those taking a placebo saw an overall 36 percent increase in inflammation-linked IL-6 by the end of the study.

"This finding strongly suggests that inflammation is what's driving the changes in the telomeres," Kiecolt-Glaser said.

The researchers concluded that this combination of powerful and healthy effects suggests that taking omega-3 supplements could represent a rare, single nutritional intervention that has potential to provide extraordinary health benefits. Specifically, taking these supplements regularly may lower the risk for a host of diseases associated with aging, including coronary heart disease, Type 2 diabetes, arthritis and even mind-robbing Alzheimer's disease.

Common ingredients in anti-wrinkle creams

The effectiveness of anti-wrinkle creams depends in part on the active ingredient or ingredients. Here are some common ingredients that may result in slight to modest improvement in the appearance of wrinkles.

  • Hydroxy acids. Alpha hydroxy acids, beta hydroxy acids and poly hydroxy acids are synthetic versions of acids derived from sugar-containing fruits. These acids are exfoliants — substances that remove the upper layer of old, dead skin and stimulate the growth of smooth, evenly pigmented new skin. Because hydroxy acids increase your susceptibility to sun damage, always wear sunscreen during use and for at least one week afterward.
  • Coenzyme Q10. Coenzyme Q10 is a nutrient that helps regulate energy production in cells. Some studies have shown reduction in fine wrinkles around the eyes with no side effects. Other studies show that application before sun exposure protects against sun damage.
  • Copper peptides. Copper is a trace element found in every cell. In products applied to the skin, it's combined with small protein fragments called peptides. Copper peptides enhance wound healing. They also stimulate production of collagen and may enhance the action of antioxidants.
  • Kinetin. As a plant growth factor, kinetin may improve the appearance of wrinkles and uneven pigmentation with minimal irritation. It's unclear how it works, but it may help reduce wrinkles by helping skin retain moisture and by stimulating the production of collagen. It may also be a potent antioxidant.
  • Tea extracts. Green, black and oolong tea contain compounds with antioxidant and anti-inflammatory properties.     Consider these points when judging the merits of using a wrinkle cream: 

  • Cost. Cost has no relationship to effectiveness. A wrinkle cream that's more costly may not be more effective than a less-costly product.
  • Latest research. Research suggests that certain ingredients may improve the appearance of wrinkles. However, most anti-wrinkle creams haven't been subjected to the comprehensive, objective research required to prove this benefit.
  • Lower doses. Nonprescription wrinkle creams contain lower concentrations of active ingredients than do prescription creams. So results, if any, are limited and usually short-lived.
  • Daily use. You'll likely need to use the wrinkle cream once or twice a day for many weeks before noticing any improvements. And once you discontinue using the product, your skin is likely to return to its original appearance.
  • Side effects. Some products may cause skin irritation, rashes, burning or redness. Be sure to read and follow the product instructions to limit possible side effects.
  • Individual differences. Just because your friend swears by a product doesn't mean it will work for you. People have different skin types. No one product works the same for everyone.

Your anti-wrinkle regimen

An anti-wrinkle cream may lessen the appearance of your wrinkles, depending on how often you use it, the type and amount of active ingredient in the wrinkle cream, and the extent of the wrinkles you want to treat.
But if you want to take the guesswork out of your skin care regimen, try these more reliable ways to improve and maintain your skin's youthful appearance.
  • Protect your skin from the sun. Exposure to ultraviolet (UV) light speeds up the natural aging process of your skin, causing wrinkles and rough, blotchy skin. In fact, sun exposure is the No. 1 reason for signs of aging in the skin, including uneven pigmentation. Protect your skin — and prevent future wrinkles — by limiting the time you spend in the sun and always wearing protective clothing and hats. Also, use sunscreen on exposed skin when outdoors, even in winter.
  • Choose products with built-in sunscreen. When selecting skin care products, choose those with a built-in sun protection factor (SPF) of at least 15. Also, be sure to select products that are broad spectrum, meaning they block both UVA and UVB rays.
  • Use moisturizers. Dry skin turns plump skin cells into shriveled ones, creating fine lines and wrinkles long before you're due. Though moisturizers can't prevent wrinkles, they can temporarily mask tiny lines and creases.
  • Don't smoke. Smoking causes narrowing of the blood vessels in the outermost layers of your skin. It also damages collagen and elastin — fibers that give your skin its strength and elasticity. As a result, skin begins to sag and wrinkle prematurely.
  • Eat a healthy diet. There is some evidence that certain vitamins in your diet help protect your skin, particularly vitamins A, C, B3 and E. More study is needed on the role of nutrition.

Anti-aging creams are predominantly moisturiser based cosmeceutical skin care products marketed with the promise of making the consumer look younger by reducing visible wrinkles, expression lines, blemishes, pigmentation changes, discolourations and other environmentally (especially from the sun) related conditions of the skin. A comprehensive grading scale for anti-aging of the skin has been validated and categorizes skin aging as: laxity (sagging), rhytids (wrinkles), and the various categories of photoaging, including erythema (redness), dyspigmentation (brown discolorations), solar elastosis (yellowing), keratoses (abnormal growths), and poor texture.[1]

Despite great demand, many such products and treatments have not been proven to give lasting or major positive effects. One study found that the best performing creams reduced wrinkles by less than 10% over 12 weeks which is not noticeable to the human eye.[2] Another study found that cheap moisturisers were as effective as high-priced anti-wrinkle creams.[3]However, recent studies at Manchester University showed that some ingredients have an effect.[5] Traditionally, anti-aging creams have been marketed towards women, but products specifically targeting men are increasingly common.[7]


Ingredients

As well as more conventional moisturising ingredients, anti-aging creams usually contain anti-aging ingredients such as:
  • Retinol (for instance, in the form of retinyl palmitate). In various formulations it has been shown to reduce fine lines and pores.[8]
  • Epidermal Growth Factor, made of 53 amino acids to stimulate cell renewal and Collagen production in the skin and strengthen elasticity and structure. The discovery of Epidermal Growth Factor won Dr. Stanley Cohen and Rita Levi-Montalcini a Nobel Prize in Physiology and Medicine in 1986. In various research Epidermal Growth Factor has been shown to reduce fine lines, wrinkles and sagging.[9] It also has healing (wounds and burns) and anti-inflammatory properties when applied to skin.[10]
  • Alpha hydroxy acids (AHAs) and beta hydroxy acids or other chemical peels. These help to dissolve the intracellular "glue" that holds the dead cells together on the skin. The use of this type of product on a daily basis gradually enhances the exfoliation of the epidermis. This exposes newer skin cells and can help improve appearance. AHAs may irritate some skin, causing redness and flaking.
  • Peptides, such as Argireline (acetyl hexapeptide-3), Matryxil, and copper peptides.
  • Coenzyme Q10
  • Anti-oxidants are substances that may protect cells from the damage caused by unstable molecules known as free radicals.
  • Sunscreens. A high level of UVA protection is recommended as UVA radiation is associated with aging effects such as wrinkles.[11]
  • Vitamin C. Supposedly one of the most effective and commonly included ingredient in wrinkle creams, also thought to help the healing process.
The effects of these ingredients depends on their concentration and mode of application. Many skin care companies recommend using a treatment programme which may combine these ingredients. For example, AHAs can make the skin more vulnerable to damage from the sun, so the increased use of sunscreens is often recommended.




Alternative approaches

Traditional moisturisers or sunscreens may provide many of the same benefits as some anti-aging creams. Facial toning, either by hand, using hands-free devices or through electrostimulation of the facial muscles, is thought by some to reduce wrinkles.

Mechanical exfoliation is an alternative to chemical peels using ingredients such as crushed apricot kernals, salt, sponges or brushes. Advertising sometimes presents anti-aging creams as an alternative to plastic surgery and botox injections, which may be more costly and invasive treatments.
